We know how challenging it can be to squeeze in a doctor’s visit in today’s fast-paced world, so we’re bringing health checkups directly to you. Through our partnership with Catapult Health, we’re once again offering onsite checkups—a quick and convenient way to take care of your health during your busy day.

Here’s how Catapult works:
1. Register and schedule your appointment in under two minutes.
2. Arrive at your selected location for your 30-minute appointment.
- We’ll take a few measurements and a small sample of blood using our revolutionary
virtually painless device.
- You will complete a follow-up consultation with a certified Nurse Practitioner to review your results.
3. Earn your Incentive: Participants must complete a Catapult Health Checkup or submit a PCP form
between October 1, 2024, and September 29, 2025. Participants will earn a premium credit of
$120 ($10 per month) beginning in January 2026.

Who can participate?
All OGB primary plan members (actives and retirees/surviving spouses) who are enrolled in the following LA Blue plans are invited to benefit from this terrific service at NO COST: Pelican HRA, Pelican HSA, Magnolia Local, Magnolia Local Plus, and Magnolia Open Access. Those enrolled in the Medicare Advantage plan are not eligible to participate. The checkups are offered as part of the Live Better Louisiana wellness program for OGB members.
Primary Care Provider Form
The 2025 PCP Form must be submitted to Catapult Health between October 1, 2024, and September 29, 2025.

Please Note: Participation in this health checkup is not recommended for pregnant women or those who have had a double mastectomy with bilateral lymph node removal. If this pertains to an eligible associate, we encourage them to continue routine care with their healthcare provider and submit a Medical Exemption PCP Form 2024-2025.

- All time should be entered no later than the Sunday which is the last day of the pay period, for those that may work on the weekends.
- Monday before 12:00 noon, you should certify your time in LEO CATS Time.
- Supervisors approve after 1:00 pm. Supervisors please seriously review dates and times in your audit process before you Approve. This will eliminate any errors in time and HR having to make corrections and recouping any overpayments.
